Convert initial APHL criteria into DiD config

What needs to be done

We have some bullet points from APHL on what a reasonable baseline would be for actionable changes. We need to convert that into a config usable by the app.

Why it needs to be done

So that the config / rules that DiD is working with is representative of reality.

Acceptance Criteria

• All bullet points provided by APHL (listed below in additional context) are reflected in a rule
• Any relevant rules are checked into the repository

Actionability according to APHL:

• Everything in Entries / clinicalStatements
• Within recordTarget (patient)
  • Deceased status (false →true)
  • Date of birth (corrections)
  • Patient name

Should this config adhere to the spec defined in #54, or do you foresee this ticket will include changes to the config spec as well?
