Finish frontend UI for EP worfklow

• Minor adjustments to the 'stepper' on the record details page, ensure it is clear that steps have been completed and they don't need any further action

• Find a nicer way to present the request/submission details at the top of the EP request page

• Move the approved report number to a more logical place on the request page
