chore(blog): generate missing cover images and add subtitles

- Add subtitles to 33 posts missing them
- Generate cover.png for 33 posts missing covers
- Replace 8 cover.jpg files with generated cover.png
- Remove redirect_from rule from one post
- Fix generate-cover.js to handle Windows line endings
