feat(memory): add Intelligence v2 — context state machine + knowledge tiers

4 new modules (intelligence_v2, claim_graph, context_packer, impact_graph),
9 new tables, 9 new MCP tools implementing human-sovereign knowledge
management with opt-in enrichment, typed SPO claims, governance gates,
role-specific context packs, and downstream impact propagation.
