1. if there are any architecture, new features or something else which should change the previous document like ADR, PRD, or any other fundamental document, should ask user for permission to change the document. MUST check with the user. the agent must do the update document flow proactively if identified something might change the docs in case miss some direction / decisions during the conversation.
2. a document work flow should be design if user mentioned: i want to add some new feature like....
