Fresh agent pane header bar should show directory, branch, remaining%, and icon list

Description
The header bar for a fresh-agent chat pane does not show the same project/worktree metadata that a CLI terminal pane shows. In the sidebar the project is visible as freshell, but the top tab/header area only shows the generated session title and the provider badge (Freshopencode).

Observed

• Sidebar label shows freshell.
• Top header only shows the generated session title + Freshopencode badge.

Expected
The fresh-agent header should mirror the CLI header pattern: directory (branch) remaining% and an icon list, plus the "open terminal pane" affordance specific to fresh agents.

Environment

• Dev worktree at /home/dan/code/freshell/.worktrees/dev
• UI served on http://localhost:5173
