feat(cli): update CLI help examples

- Updated README to clarify CLI usage and examples
- Added explicit root command examples for common actions
- Documented an explicit planner intent example in CLI help

this change updates the cli scope by enhancing command line help and
documentation. it adds representative examples to the root command help
and README, including an explicit intent example for commit grouping to
guide users.
