Codex review no response all the time in claude code VS Code plugin

Recently codex review are easy to stuck when review the changes, when I am using version 1.0.4.

Before when using 1.0.2 no matter how many changes have been made, Codex review always returned results.

So I need to divide the reviewing task into 2 sperate jobs.

+1 on the v1.0.4 regression. Hit this consistently today across 3 unrelated PRs (different diff sizes, different surfaces) on macOS 25.4 / Claude Code 2.1.91 / Codex CLI 0.130.0. Same pattern every time: .log shows active Running command: / Command completed: pairs for 5–12min, then goes silent. No further log entries, review never reaches task_complete, status --all --json shows running indefinitely.

Possible overlap with #345 — I trigger --background from a Claude Code Agent subagent that exits right after the trigger returns. If the subagent's session-end is killing the Codex background, it'd match the "silent forever" symptom even though I'm on macOS (not Windows, where #345 was reported).
