Edit styling for new student form

Summary

As an admin, I want to add new student information by adding volunteer, staff contact, guardian information, etc.

Things to Consider

The form is already implemented with most of the required fields. Need to additional information and align styling according to the design guidelines.

Criteria for Completion

• Format all the field size and fonts (Some are uneven) Eg: state field. Make sure the buttons are formatted and sized correctly.
• Add referring agency field, student school name
• Assign volunteer and staff contact fields to be uniform and similar to the screenshot attached. Have a search icon in the field and display the selected name below the field
