Add support for app version and OS in the formatting

Reasoning:

• Google analytics can segment on meta-data, but its cumbersome to organise
• Google analytics often group similar exceptions, if we include os and version we can easily see what versions has issues etc.
• Omitting date would also be better because then exceptions gets grouped more easily

Example:
