Adds gated read-only LSP tools for diagnostics, document symbols, goto definition, find references, and hover for Rust and TypeScript providers. Tools are workdir-scoped, classified as filesystem-read, and go through the existing gate/audit path. Preserves MCP behavior, replay behavior, trust gates, approvals, retry policy, and continuation semantics.
