fix(index): restore the em dash in the catalogue header - #32

INDEX.md is regenerated from ADR frontmatter; the header sentence is retained prose, not a separate generator string. Commit 96032f5 stored U+2014 as cp1252 mojibake, and later row regenerations left that line in place. The catalogue uses the same em dash as the rest of the repo.

96032f5rewrote the file and stored every em dash as that mojibake. Later regenerations rebuilt the table from frontmatter titles, which already use a real em dash, and kept the static header.Rebuilding all 63 rows from
adr/frontmatter matches the current table. The header now uses U+2014, the same character as the empty Depends on cell and the rest of the en-GB catalogue. A byte scan of the tree found no other copy of this mojibake (em dash, en dash, quotes, ellipsis, or nbsp).A one-character restoration skips a plan item and an ADR revision.
