Every review-carried record carries a control, and a test keeps it that way - #423
Closes#420. Part 1 (the --review --drive runner) landed in #422.
What changed
28 new controls. Every review-carried record now has a catches/misses pair in policy/upheld.toml, and each one names a real file or function in this repo. Each misses sits next to its catches (same file, same feature) without violating the record, so a reviewer that matches words instead of the claim fires on it.
A test keeps it at 31 of 31. The new test_every_review_carried_record_here_carries_a_control fails if review-controls.json lists any uncontrolled record. A record added to the review tier now needs a control in the same PR. That is a new rule for record authors, so flag it if you'd rather not have it.
Removed:test_the_denominator_is_printed_on_a_run_that_found_nothing_wrong, because it asserted this repo still has uncontrolled records. The uncontrolled_note unit test still checks that the line names the records.
The ROADMAP item for controls is removed, since it is done.
Evidence
Reviewer: a wrapper around claude -p --model haiku, not committed (uphold ships no model client).
Four Sonnet subagents drafted 7 controls each and tested each side twice.
In the first full --drive run, 29 of 31 held.
Every catch and every miss was then run 10 times. Weak catches were rewritten until they hit 10/10 or 15/15: end-to-end-principle, least-astonishment, mechanism-policy-separation, confused-deputy, reversible-decisions, observability. No miss fired in any run.
The final full run:
pass@1 1.00; pass^3 31/31 control(s)
false positives 0.00 over 31 control(s) with a misses
Caveat: at Haiku's per-plant miss rate before the rewrites (about 2%), one run in a few still showed a single control at 2/3. The rewrites raised the weakest catches, but a later model or reviewer prompt can bring that back. --drive exists to show it when it happens.
…at way
The 28 records with no control now have a `catches`/`misses` pair in
policy/upheld.toml, each naming a real file or function here. Each
`misses` sits beside its `catches`, so a reviewer that matches words
rather than the claim fires on it.
A self-application test now refuses a non-empty `uncontrolled` list, so
a record added to the review tier without a control fails the suite. The
ROADMAP item is removed.
test_the_denominator_is_printed_on_a_run_that_found_nothing_wrong is
removed: it asserted this repository prints a non-empty uncontrolled
list, which is no longer true. uncontrolled_note's own unit test still
pins that the line names the records, and
test_every_review_carried_record_here_carries_a_control replaces it.
Every catch was run 10 times against claude haiku through
`--review --drive`; the six that fell short of 10/10 were rewritten
until they hit it. No misses fired.
Closes#420.

Summary
Control scenarios and catalog policy/upheld.toml, review-controls.json
The policy adds paired catch/miss scenarios for review-carried principles. The JSON catalog adds control descriptions and changes uncontrolled to an empty array.
Coverage assertion and self-application test policy/upheld.toml, tests/test_review.py, ROADMAP.md
The policy comments state that every review-carried record has a tested control. The test checks for an empty uncontrolled list and no uncontrolled-record notice. The roadmap item for adding controls is removed.

Explanation
The title states the main change: every review-carried record in policy/upheld.toml now has a control, and a new test enforces this. The raw summary confirms the added control pairs in upheld.toml and review-controls.json, the emptied uncontrolled list, and the new self-application test. The objectives confirm the same scope. The title is specific, single-sentence, and avoids vague terms. It is 13 words and 75 characters, which is slightly long but still readable.

Explanation
Issue #420 has three coding objectives. The PR adds controls for the remaining review-carried records in policy/upheld.toml and exports 31 controls with "uncontrolled": [] in review-controls.json. The new test_every_review_carried_record_here_carries_a_control runs --review and asserts that no uncontrolled record remains. The reviewed head also contains the caller-supplied reviewer protocol, drive, and DEFAULT_RUNS = 3 in scripts/review.py. These existing changes provide per-record scoring and repeated-run support. The PR adds the missing controls and enforcement without requiring a duplicate runner implementation.
Full details: Out of Scope Changes check
Explanation
The changed files stay within issue #420. The changes add control fixtures, export and enforce their coverage, update the related self-application test, and remove the completed ROADMAP item. The removed denominator test is obsolete because the review tier now has no uncontrolled records. No unrelated production feature or unrelated test change is shown.

catches = "Let prevent_public_push in src/guard/push.rs accept allow-list entries written as regular expressions, per-rule fallback commands and an \"ask the forge, else a script\" chain, all decided inside the one function that refuses a push."
misses = "Add unit tests to src/guard/push.rs showing that a destination under another owner and a destination with differently cased owner are both refused, changing no code outside the test module."

Negative control expects wrong refusal
The economy-of-mechanism miss says differently cased owners should be refused, but src/guard/push.rs accepts allowed owners without regard to case. For an allowed owner, the proposed test would fail despite the guard working correctly. That makes this a misleading example for checking reviewer accuracy.
Make the miss test that an unrelated owner is refused and a differently cased allowed owner is accepted, then regenerate review-controls.json.

Fixed in 70df829: the miss now says an undeclared owner is refused and a declared owner in a different case is accepted, matching eq_ignore_ascii_case. Against haiku it fired 0/10.

…and a miss may sit beside its catch in another file
prevent_public_push compares owners with eq_ignore_ascii_case, so a
declared owner in a different case is accepted, not refused; the miss
said otherwise. Against claude haiku the corrected miss fired 0/10.
The comment above the controls said a miss uses the same file and path.
Several pairs sit in the same feature across two files, so it now says
the same feature or decision, often the same file.
