…equirements Add a CCB change request template page alongside the existing RAT requirement template under docs/guides/. The template covers change type (single-select), original baselined requirement, new requirement, itemized change points with impact, workload delta, delivery timeline delta, risk and rollback, and the CCB review conclusion. Register the page in docs/guides/_index.md and docs/_index.md; both validators pass (the pre-existing scheduler/rules.md split-line error is unrelated to this change).

Add a CCB change request template page for baselined requirements, alongside the existing RAT requirement template under
knowledge/repos/vllm-omni/docs/guides/.The template covers: change type (single-select), original baselined requirement (verbatim from the approved RAT), new requirement description, itemized change points with impact analysis, workload delta, delivery timeline delta, risk and rollback, and the CCB review conclusion.
Why
Requirement changes after baseline review currently have no documented template in this repo; the RAT template only covers first-time requirement intake.
