Every document an agent writes under TASKS_DIR must now be either an ultra-short index that routes by theme, or an annex that holds the content of a single theme in loadable sections, so context is read on demand.

  • add the doc-pattern core skill (installed with every profile, mirrored to Codex) as the authority on shape, size budgets, splitting, exemptions; it follows its own pattern: short SKILL.md plus three annexes
  • state the obligation in base-rules.md, so it reaches PROJECT_RULES.md whatever the installed selection, and route to it from project-workflow
  • restate it where documents are written: tasks, bugs, memory, knowledge, continuity, exploration, learning, reporting rules and their skills, plus the quality closing gate and the project-notes example
  • add the index declarative check: it verifies index links resolve and that every index table row routes through a link instead of carrying content; replaces the narrower links checks on owned indexes
  • document the pattern in README and docs/modules.md, and cover skill mirroring, composed rules and the new check in tests

Every document an agent writes under TASKS_DIR must now be either an
ultra-short index that routes by theme, or an annex that holds the content
of a single theme in loadable sections, so context is read on demand.

- add the `doc-pattern` core skill (installed with every profile, mirrored
  to Codex) as the authority on shape, size budgets, splitting, exemptions;
it follows its own pattern: short SKILL.md plus three annexes
- state the obligation in base-rules.md, so it reaches PROJECT_RULES.md
whatever the installed selection, and route to it from project-workflow
- restate it where documents are written: tasks, bugs, memory, knowledge,
continuity, exploration, learning, reporting rules and their skills,
plus the quality closing gate and the project-notes example
- add the `index` declarative check: it verifies index links resolve and
that every index table row routes through a link instead of carrying
content; replaces the narrower `links` checks on owned indexes
- document the pattern in README and docs/modules.md, and cover skill
mirroring, composed rules and the new check in tests
