chore: remove committed codegraph.db binary artifact

What

• Deletes .codegraph/codegraph.db (1.1 MB SQLite index generated by a local code indexing tool, committed by accident)
• Adds .codegraph/ to .gitignore so it cannot be committed again

Why

LFX Insights control OSPS-QA-05.02 fails because of this unreviewable binary artifact:

Unreviewable binary artifacts found in the repository: codegraph.db

The file has no value in version control and cannot be reviewed in diffs.
