Remove legacy review optimization chain

• Remove source-local legacy AI review, OpenAI secondary review, monthly optimization, experiment validation, and ccbot feedback/auto-merge chain
• Keep monthly review routed directly through CryptoCodexAuditBridge
• Update docs/tests to assert the legacy workflows are gone
