Looks good, Courtney. I added the key and adjusted the layout of the GoogleForm a bit to match our other forms.
One thing below.

One question: Is this supposed to be yes for a name or yes for anonymous?

I talked to Rachael and she would like it on a separate page. Do you mind moving the form to a separate page (in the same directory) and linking to it from this one (like we do on the mutua11y forms for mentoring / protogés) ?
