feat: Github templates

Description

Adding github templates for pull requests so we can standardize and communicate better. We want to move to semantic PR naming (will need @sandeep-aot to add as a prerequisite check).

Checklist

• I have read the CONTRIBUTING doc
• I have performed a self-review of my own code
