feat: complete CRP management workflow

Summary

• Complete named Provider management, model discovery/cache, compatibility testing, activation, lifecycle controls, and expanded human CLI status/help.
• Add safe Codex provider-transition history repair and strict readiness coordination while preserving the fixed Codex provider and loopback proxy address.
• Replace the legacy Web surface with a Vite + React management console, aggregate Metrics, provider quick actions, read-only session recovery, responsive accessibility coverage, and deterministic English-first language behavior.
• Add authenticated graceful Supervisor shutdown, a dedicated Web Exit CRP flow, recoverable fixed state cleanup, and hidden Supervisor/Worker windows on Windows.
• Ship MIT licensing and the package/build verification required for the release candidate.

• GitHub Actions: Linux, macOS, Windows, and Release Preflight all passed for 7425df4.
