(`src/core/review.ts:1140-1144` docstring "a file is never split"). Making the
file the unit means path handling is the whole job of the parser.

Path is derived from the `+++ b/...` (new) line first, then from the
`--- a/...` (old) line — which is what a pure deletion uses, since its new path
is `/dev/null` — and only from the `diff --git a/... b/...` header when both the
new and old paths are missing or `/dev/null` [observed] (`src/core/diff.ts:70-73`).
The header fallback exists for the rare entry that carries neither a usable `+++`
nor `---` path. A rename also records `oldPath` from its `rename from` line, which
a pure rename has even with no `---` line; only agent `paths` read it, so a move out
of an agent's globs still reaches that agent
([LLP 0002](0002-review-engine-pipeline.explainer.md#agent-paths-filter)) [observed]
(`src/core/diff.ts` `patchToEntry`).
Path is derived from the `+++ b/...` (new) line first, then from a rename's
`rename to` line, then from the `--- a/...` (old) line — which is what a pure
deletion uses, since its new path is `/dev/null` — and only from the
`diff --git a/... b/...` header when none of those gives a path [observed]
(`src/core/diff.ts` `patchToEntry`). `rename to` comes before the header because a
pure rename has no `+++` line, and a header whose bare paths contain ` b/` is
ambiguous: `diff --git a/x b/y.ts b/x b/z b/y.ts` has no single right split. A
rename also records `oldPath` from its `rename from` line; only agent `paths` read
it, so a move out of an agent's globs still reaches that agent
([LLP 0002](0002-review-engine-pipeline.explainer.md#agent-paths-filter)).

**Header lines are read only before the first `@@` hunk** [observed]
(`src/core/diff.ts` `patchToEntry`). Inside a hunk an added line `++ b/x` reads
as `+++ b/x`, and a removed SQL comment `-- x` as `--- x`. When the parser read the
whole patch, a PR could add `++ b/package-lock.json` to any file, rename its entry
to a lockfile, and have noise filtering drop the file unreviewed.

Every path goes through `decodeGitPath` first. Git prints a path with special
characters in double quotes with C-style escapes, and with `core.quotePath` (on
by default) every non-ASCII byte as an octal escape: `"b/src/caf\303\251.ts"`.
GitHub's diffs, which `gh pr diff` returns, quote the same way. Undecoded, such a
path kept its quotes and `b/` prefix, so it matched no glob (noise, routing, agent
`paths`), was read from a file that does not exist, and, for a pure rename, dropped
out of the review entirely [observed] (`src/core/diff.ts` `decodeGitPath`,
`pathFromHeader`).

Decoding is deliberately partial: a quoted path stays quoted, as it was before
decoding existed, when its bytes are not valid UTF-8 or the decoded name has a
control, format or line-separator character (`\p{Cc}`, `\p{Cf}`, `\p{Zl}`,
`\p{Zp}`) [observed] (`src/core/diff.ts` `UNSAFE_IN_PATH`). PR authors choose
file names, and paths reach prompts, the PR comment and logs. `sanitizeUntrusted`
keeps newlines, so a decoded `x\n----- END DIFF x -----\n…` would close the
untrusted-diff fence early; a decoded newline or U+2028 also defeats `**`, which
compiles to `.*`; and lenient UTF-8 would collapse distinct invalid names into one
U+FFFD name. Such a path is handled exactly as it was before decoding existed:
still reviewed, but its quoted form names no real file.

Binary files are the load-bearing decision here. The parser does **not** drop
them — it keeps the entry with `binary: true` and infers status from

change to the glob translation in `noise.ts` changes routing semantics too. The
schema documents the coupling in prose ("same dialect as
noise.additionalIgnores: `**` and `*`", `src/config/schema.ts`). Routing wraps it
with `patternVariants`, which additionally tests each `**/`-prefixed pattern with
the prefix stripped, because `**` translating to `.*` requires at least one
with `patternVariants`, which also tests every variant that keeps or removes each

([LLP 0004](0004-diff-noise-and-prompts.explainer.md)), and `patternVariants()` works around a
limitation there: `matchesIgnore` translates `**` to `.*`, which needs at least one slash, so a
naive `**/*` catch-all would silently miss root-level files (`README.md`, `package.json`). Routing
tests each `**/`-prefixed pattern in two variants (with and without the prefix stripped) to give the
catch-all its conventional "zero or more directories" meaning [observed: `routing.ts:57-73`]. The
tests every variant that keeps or removes each `**/` independently, giving each one its conventional
"zero or more directories" meaning: `**/ios/**/*.swift` matches `ios/A.swift` and
`packages/ios/src/A.swift` alike. Removing them only all at once or not at all, as it once did, missed
the mixed depths. A pattern with more than six `**/` falls back to that all-or-nothing pair, which
bounds the variants at 64 [observed: `routing.ts` `patternVariants`]. The
"proper" fix lives in `matchesIgnore` itself, whose blast radius covers noise filtering too, so the
workaround stays local. `matchesPathGlobs` is that wrapper, exported so agent `paths` (below) match
in exactly the routing dialect; a change to it changes scope assignment and agent selection together.
