Rename InsufficientScope to MissingScopeException - #37

- InsufficientScope becomes MissingScopeException (old names stay as aliases) - Exception classes take an Exception suffix; AuthError is now AuthException - InvalidSessionException becomes AuthenticationException - Bad password on the password grant raises InvalidCredentialsException - A request with no credential raises AuthenticationException - Tests and docs follow the new names
