feat(blocked-prompt): relay Codex tool approval dialogs to the phone - #32

Parse codex-cli 0.166.0 shell-command and file-edit approvals into the sealed summary (command or destination, reason, numbered answers) answered with up/down + enter, which lands on No the way esc does.
Add the codex-blocked-tool-approval rule (question + confirm footer), bench rows for the three captured dialogs, bundle version 2026.11.01.1 with the matching published floor, and the README line.
