✨ Add global id types

This is an implementation of defining custom types (for pt and ob global id types) to test a possible global id specification implementation.

In the derived SQL, they all just appear as text types.

I added some additional global id types in. A couple of gaps still:

1. We are missing a few global ID types in dewrangle: FamilyMemberHistory, PlanDefinition, and MedicationStatement.
2. It may be sensible to rearrange the specimen related items such that there are not ids without a global id match. Specifically, putting them in Sample with inlined=F may be appropriate here.
