build(many): remove commonjs output (ESM-only)

Summary

• Drop the CommonJS build: each package now builds es/ only (removed --modules es,cjs from build scripts; ui-scripts build defaults to es).
• Set "type": "module" and remove the "main" (./lib) field + all require/./lib/* export entries across ~87 packages; versioned exports (./v11_6, ./v11_7, ./latest) keep their import/default/src/types only.
• Rename each package's babel.config.js → babel.config.cjs so Babel config still loads under "type": "module".

Test Plan

• Smoke-test a downstream consumer that imports InstUI via require() — those paths are intentionally gone now (breaking).
• Verify the docs app and any webpack/Cypress builds still resolve packages via the es/exports fields.
