Implementation Details
Elimination of Untrusted Client Actor Headers: Removed all inspection and trust in X-Actor-Id and X-Actor-Role. Caller identity is strictly bound to authenticated current_user.id.
Elimination of Hardcoded Fallbacks: Removed hardcoded identities and default "admin" fallback in get_effective_user_role(). Roles are computed dynamically via MembershipRepository or fail closed.
Fail-Closed Workspace Authorization: Updated resolve_authorized_workspace_id() to strictly validate bearer tokens/sessions against UserRepository and SessionRepository and verify active workspace membership. Missing auth or suspended membership returns HTTP 401 / 403.
Authoritative Server-Side Identity Derivation: Proposers and reviewers are derived strictly from server-side session context.
Authoritative Workspace Membership & RBAC Enforcement: Enforced explicit permissions (MIGRATION_READ, MIGRATION_COMPARE, MIGRATION_PLAN, MIGRATION_REVIEW, MIGRATION_APPROVE, MIGRATION_EXPORT) with membership checks on every endpoint.
Trusted Quorum Governance: Derived review approval quorum strictly from server-side governance policy (policy.required_approval_quorum), rejecting untrusted client-supplied quorum values.
Durable, Transaction-Safe Database Persistence: Implemented relational models (MigrationReviewRecord, MigrationReviewDecisionRecord, MigrationAuditEventRecord) in dera_migration_reviews, dera_migration_review_decisions, and dera_migration_audit_events with transaction-safe unit of work in MigrationGovernanceRepository.
Authentic Risk-Report Fingerprint Linkage: Added source_risk_report_fingerprint to SqlMigrationArtifact, forwarded from plan_report.risk_report_fingerprint and bound to the review state.
Exhaustive Security Integration Tests: Implemented 7 end-to-end security test cases verifying fail-closed handling for unauthenticated requests, spoofed actor headers, separation-of-duties self-approval, cross-tenant access, revoked memberships, artifact tampering, and durable DB persistence.
Frontend Integration & Typed API: Added typed methods in api-client.ts, wired MigrationDashboard.tsx to real endpoints with distinct keys per workspace, and mapped response structures accurately.
Testing
Relevant tests passed (python scripts/development/dev.py all or test)
Strict Non-AI Policy: Verified zero generative AI, LLM, or external inference APIs are introduced.
Database Safety: Verified all database connector operations enforce read-only introspection without data modification.
Credential Protection: Confirmed that no production credentials, passwords, or .env secrets are committed.
Least Privilege: Validated that permissions and access boundaries are minimized.
Breaking Changes
No breaking changes.

Merge Risk: High · up to 35a8f
Governance review persistence will likely fail on databases built with the Alembic migration, so the new workflow is unusable in a migrated deployment. The SQL review view can also tell a reviewer a destructive migration has no destructive statements. Fix the migration schema alignment and the safety data before merging; the identity-spoofing and admin-fallback issues are resolved.
Security architecture risk: High · up to 35a8f
The new workflow has material authorization and governance-state gaps. Database-free operation accepts unverified credentials for workspace access, viewer permissions do not restrict several migration operations, and approval state can diverge from committed audit evidence. The database migration also disagrees with the application’s persistence model. The inspected paths generate SQL rather than execute it, which limits the demonstrated impact.
Retained concerns
High · security · observed: When db_session_factory is absent, workspace authorization accepts any nonempty bearer or session-cookie token and returns the caller-selected workspace UUID without validating credentials or membership. This removes the base revision’s required database dependency and exposes the new non-governance migration routes, plus existing consumers of this dependency, to unverified callers. Disclosure depends on data being present in an externally reachable stateless service. A forged token alone does not satisfy the separate authenticated-user guard on decisions or exports.
Medium · security · observed: The new compare, plan, and SQL-compilation routes enforce the shared SCHEMA_READ boundary rather than their migration-specific permissions. An active viewer therefore reaches operations excluded by MIGRATION_COMPARE and MIGRATION_PLAN policy, including compilation responses containing forward and rollback SQL. Workspace scoping remains intact in the database-backed path, and this does not establish migration execution or privileged governance access.
High · security · inferred: Approval and audit objects are mutated in the shared cache before database flush and commit. On persistence failure, endpoint rollback does not restore or evict those objects, and subsequent retrieval returns cached state before consulting durable records. Consequently, a failed approval transaction can remain externally visible as APPROVED and satisfy the service’s approval-status check for a later export attempt without corresponding committed approval evidence. Successful export after such a failure is a derived risk, not an executed result.
High · reliability · observed: The new deployment migration and application persistence contract disagree. The migration creates review_id, whereas the ORM and repository query review_id_str; the ORM also requires migration_plan_fingerprint and target_dialect columns absent from the review table definition. An installation provisioned through this migration cannot support the repository’s review queries and writes, preventing the intended durable approval and audit workflow. This is a source-level compatibility failure; no migrated database was exercised.
Security review details
Security Blast Radius
inferred — In a reachable, populated stateless deployment, an unverified caller knowing a workspace UUID can select that workspace and access data exposed by consumers of the shared dependency. In normal database-backed operation, the demonstrated viewer permission gap stays within an active membership’s workspace. Inspected compilation and export outcomes are SQL text and governance records, not execution against tenant databases.
Security Findings and Attack Paths
observed — Two introduced authorization paths are supported by source: supplying an arbitrary nonempty token bypasses workspace verification without a database, and an authenticated viewer passes SCHEMA_READ into comparison, planning, and compilation without the corresponding migration permission checks. Neither path demonstrates a forged authenticated reviewer identity.
Trust Boundaries and Controls
observed — Database-backed authentication rejects invalid, revoked, expired, or inactive-user sessions and inactive workspace membership. Decision and export handlers additionally require authenticated request-state identity and enforce MIGRATION_APPROVE or MIGRATION_EXPORT when a database session exists. Their admin defaults without a database are conditional weaknesses, not proof that an arbitrary token can approve or export.
Resilience and Maintainability Implications
inferred — Governance authority is split between mutable cached objects and durable records. Besides rollback divergence, repository reads have no row lock or version condition, while audit predecessors are selected from cached event lists. Multiple workers can therefore evaluate transitions against different histories; the exact concurrency outcome depends on deployment and database isolation that were not supplied.
Hardening Proposals
proposed — Make committed governance state authoritative: coordinate decision, status, and audit append in one serialized or version-checked transaction, publish cache updates only after commit, and define retry and artifact-recovery semantics. Authorization should fail closed whenever the authoritative identity or membership store is unavailable.

require the requested number of approvals rather than a hard-coded quorum of
one.

membership repository errors propagate instead of swallowing them.
- Around line 362-365: Update the requester identity derivation around
requester_id to use only current_user.id, and reject unauthenticated requests
with 401 instead of falling back to a header or hard-coded identity. Apply the
same actor derivation to the related reviewer_id and user_id paths; derive
authorization roles only from the membership record, never X-Actor-Role or an
admin fallback.

context cannot bypass workspace authorization. Allow unauthenticated fallback
only behind explicit test-only configuration, and preserve the existing
authorization flow for authenticated requests.

database_name and structural_fingerprint to
ReverseEngineeringService.list_snapshots, mapping the existing database_id and
fingerprint query values respectively; keep the endpoint query parameter names
unchanged.
Review comments at @apps/web/src/app/(app)/migrations/page.tsx:
- Around line 119-136: Add distinct keys to the two MigrationDashboard branches
so switching between demo and live mode remounts the component and reapplies the
appropriate initial state. Make the live key include currentWorkspace?.id so
changing workspaces also resets the dashboard.
Review comments at @apps/web/src/components/migrations/MigrationDashboard.tsx:
- Around line 193-217: Update the normalizedSql mapping in the migration
pipeline to use the actual MigrationSqlArtifactResponse fields and avoid
accessing unavailable statement data; use defaults for statement counts where
the response provides no statements. Also align the review-data mapping around
reviewRes with the actual GovernanceReviewResponse fields, removing assumptions
about fields that response does not provide.
Review comments at @apps/web/src/lib/api-client.ts:
- Around line 846-849: Update compileMigrationSql to send target_dialect,
submitMigrationReview to use the GovernanceReviewSubmitRequest fields including
required title, and recordMigrationDecision to send decision_type and omit
actor_role. Define request and response types from the backend schemas, replace
Promise<any> for these API methods, and update their callers to use the matching
request shapes.
Review comments at @docs/audit/PHASE_1_9_FINAL_COMPLETION_REPORT.md:
- Line 31: Update the audit link in the completion report to use a
repository-relative Markdown link instead of a file:/// URL, pointing to
PHASE_1_9_MASTER_COMPLETION_MATRIX.md.
- Around line 49-55: Update the Verification Evidence section to make each
reported metric auditable: identify its command, commit, timestamp, and
validation scope, and distinguish historical results from the final
qualification run. Reconcile the backend test totals and documentation-file
counts, or clearly label their differing scopes; ensure the final sign-off
points to one consistent, verifiable qualification set.
Review comments at @docs/audit/PHASE_1_9_GAP_REMEDIATION_PLAN.md:
- Around line 16-25: Update PHASE_1_9_INITIAL_REVALIDATION.md and any remaining
references to use /api/v1/migrations/ as the migration API contract instead of
the workspace-prefixed path. State that migration handlers resolve the
authorized workspace through resolve_authorized_workspace_id.
Review comments at @docs/audit/PHASE_1_9_MASTER_COMPLETION_MATRIX.md:
- Around line 28-32: Remove the trailing whitespace reported by markdownlint in
the documentation, then rerun npm run docs:check; update the PASS statuses in
the completion matrix only if the documentation quality gate succeeds.
Review comments at @docs/audit/PHASE_9_5_GOVERNANCE_SECURITY_REVALIDATION.md:
- Line 24: Update the enforcement statement in the report to use the
implementation’s actual identifiers, `decision.reviewer_id` and
`ReviewDecisionType.APPROVE`, instead of `decision.actor_id` and
`DecisionType.APPROVED`; keep the described separation-of-duties behavior
unchanged.
- Around line 28-33: Update the `/export` fingerprint claim to match its actual
behavior: either implement recomputation from the SQL artifact content before
export and add a test that tampers with content while leaving the stored
fingerprint unchanged, or narrow the report to state only that the stored
fingerprint matches the approved review record.
