Per-upstream sub-registration methods now clear stale state when the field is at its default, making `register_upstream` a full-state idempotent setter. Also deduplicates `url().to_string()` calls, short-circuits `response_narinfo_bytes` on `Keep` mode, and removes the `set_upstream_filters` wrapper.
