fix: default Cost Lab silence assumption to 5% - #86

Add managed native-audio estimates for Azure Realtime using Pro meters and Azure Realtime Fast using an explicit Lite-tier assumption. Include comparison and CSV pricing provenance, prevent unverified BYOM substitutions, and cover pricing and scenario selection. Move Azure Realtime Fast from native speech-to-speech to the managed realtime text response plus Azure HD pipeline.
