docs: add OAuth2 authorize screenshot to Discord step 5#30

The Discord install walkthrough jumped from the "Add to server" dialog straight to the bot appearing in the server, skipping the permissions confirmation screen. Add a screenshot of that screen and correct the prose: the first dialog's button is Continue, not Authorize -- Authorize is the second screen. Source screenshot cropped to drop the trailing page-background strip below the modal.

Summary
Step 5 of the Discord channel setup ("Install the bot to your server") skipped a screen. It showed the Add to server dialog, then jumped straight to "the bot appears in your server" — leaving out Discord's permissions confirmation screen, which is where you actually click Authorize.
