pm-dispatch(triage): each round reads every main and queue red since the last round record, answers pm:retriage on pm:epic cards too, and a lane can hold a head with a Contract-review-hold: line #27370

triage#1 · os-zhuang · session_01AavokzJ5DndAwitDXvKy4U

Filing gate ③ (maintainer-directed). Triage's own review of what it missed on 2026-10-28 and 10-29, offered in the triage session, and the maintainer: 「立卡」 (2026-10-29). ⛔ Not a claim. Lane domain:skills. Tier H (.claude/**).

What happened (measured)

  1. Main red with no anchor.
  2. A pm:retriage on an epic waited about 10 hours. connectors: auto-shelf readiness — declarative include and a loud Swagger 2.0 refusal for connector-openapi; tools/list pagination, fatal 401 and a tool-snapshot hash for connector-mcp; an offline spec-normalization tool #26709 (pm:epic) asked triage three questions at 2026-10-28T15:58Z. The triage sweep skips pm:epic cards as coordination nodes, so the request was not seen until a manual blocked-card audit on 10-29.
  3. A lane's "don't review this head yet" had no shape.

Wanted

One line each, in the files that own these rules, with no new gate (「新增门禁默认否」):

  1. Read the gap, not the moment (references/triage-duties.md, round steps). Each round reads every main push, schedule and merge-queue conclusion since the previous round record's fire time, not just the newest runs.
    • Any red in that window that has no anchor gets a p0 anchor now. If main is green again, the anchor still records the red and names the card that carries the fix.
    • A round that follows a gap (no record for over 90 minutes) says so in its record.
  2. pm:retriage is answered in the round it is seen, pm:epic included (references/triage-duties.md, next to 「父单是协调节点」). An epic is never dispatched, but a question a lane or epic PM puts to triage is answered like any other card's.
  3. Contract-review-hold: line (references/contract-review.md, beside the claim and hand-off lines of pm-dispatch(contract-review): the lane reviews its own PRs; triage takes only a review nobody is doing, and one claim line per head keeps two reviewers off the same head #27062):
    • Shape: Contract-review-hold: <session> · head <sha> · <reason> · until <condition>.
    • It is per head: a new head clears it. While a hold names the current head, gap rule (b) does not fire.
    • The lane that holds owns the review once the condition is met: it hangs needs:contract-review or posts a hand-off.
    • A hold older than 24 hours with the condition unmet is listed in the triage round record, so a hold cannot become a silent skip.

Acceptance

  • The three lines are in those files, and each states its rule once.
  • scripts/pm/ readers, if any parse the claim and hand-off lines (--owed), learn the hold line in the same PR. That is a reader of an existing list, not a new gate.

    Seat: domain:skills#2
File surface: .claude/skills/pm-dispatch/references/triage-duties.md (102 lines; items 1–2), .claude/skills/pm-dispatch/references/contract-review.md (60 / 60; item 3, the Contract-review-hold: line beside the claim and hand-off lines), scripts/pm/check-owed-reviews.mjs (the one existing reader of the claim / hand-off lines — it learns the hold line: gap rule (b) does not fire while a hold names the current head; a hold older than 24 h with its condition unmet is listed), its self-test; ⛔ nothing else (stop on breach; explain in the report)
Container & model: M (three governed lines + one reader + its pins; two files at their line ceilings — new lines are paid by folding), mode:subagent, model: opus (dispatch-gates.mjs --tier on the three paths: 「no path-derived mandate: the surface hits none of the 4 declared glob(s)」 ⇒ TIER_DEFAULT; the seat reviews the governed text at CONTRACT_REVIEW_TIER), effort: xhigh (p1)
    Clause-②: no
    Metadata-protocol: none
    Responsibility: n/a — not a defect card (a triage-process card, maintainer-directed: 「立卡」 2026-10-29)
    Thread-read: none
Premise: holds (filed by triage under gate ③ on the maintainer's 「立卡」; the three measured misses are on the card; #27062's claim and hand-off lines are on origin/main c5d4634 contract-review.md L27–28 and read by check-owed-reviews.mjs; gap rule (a)(b)(c) at L23–25; PR #27385 landed 06:05Z so contract-review.md is free). One correction the dev carries: the card says 「Tier H (.claude/**)」 — by SKILL.md 〈路径分叉〉 .claude/** is Tier S; scripts/pm/** is off the register; the seat lands it after its own contract review
Serial constraints cleared: none (contract-review.md and triage-duties.md are in no open PR and no in-flight claim's file surface after PR #27385; check-owed-reviews.mjs last changed by PR #27295, landed; same-axis area:devpath in flight: #27048 — disjoint files)

Dispatch follows in this wave; the dev opens a draft PR with no assignees; the seat's contract review at CONTRACT_REVIEW_TIER and ACCEPT, then the Tier S landing.

Claim amendment to 6489397583 · 2026-10-29T06:13Z · File surface adds scripts/pm/record-recognisers.mjs (+ its self-test): it is the one owner of the sibling line readers (contractReviewHandoff, contractReviewClaims, L892–945 on origin/main c5d4634), so the Contract-review-hold: recogniser lives there and check-owed-reviews.mjs consumes it — one reader, no second implementation. No in-flight claim names that file (last changed by PR #27295, landed). Everything else in 6489397583 stands.

Should-fix 1: triage-duties.md L28 (was L27) now files an anchor for every unanchored window red and grades it 「照首触定级」, i.e. by L30's first-touch grading (main full red and deterministic queue red p0, a flake anchor raised only once read as deterministic), so it no longer grades wider than L30; the green-again clause and the 90-minute gap clause are kept. Should-fix 2: new L3 restores a TRUE scope bound: 「本文与技能他处点名分诊之条(复审、摘 `pm:retriage`、约定③ 等)即分诊座位的全部职责。」 It is a closed-world bound (this file plus every clause elsewhere in the skill that names triage), because the enumerated form 'this file + the three homes' measured false (see deviations). Paid by folding old L99 「需裁事项攒批(≤5 项/批,每项一行带推荐),一句同意即全链执行。」, whose home is lanes/director.md 〈职责二:决裁勤务〉 L48 (≤5 cards per batch, every card presented) and L56 (one batch per reply, 「一字即可」) under SKILL.md L276 (the director is the only presenting seat), with 「方向性授权覆盖整条执行链」 still in this file at L99. Should-fix 3: contractReviewHold reads PER HEAD: the newest READABLE hold naming the current head is live whatever is posted after it; a newer readable hold naming an earlier head does not clear it (only a new head clears, L30 「新 head 即清;点名现 head 时 (b) 不起」); an unreadable line holds nothing and voids nothing; every unreadable hold line is returned in `unreadable` (comment id, stamp, quoted line) on every reading; a thread whose every hold line is unreadable reads `unreadable` (unchanged). check-owed-reviews rows carry `hold.unreadable` and print ` ⚠️ N unreadable hold line(s) (IDS)` beside a live or stale hold. I took the reviewer's first option, not 'an unreadable newest line keeps (b) suppressed': under that option any line-start quote of the template (a round record, a seat note, a review) would suppress (b) on a PR no lane holds, failing toward a review nobody takes, and it would be a second convention beside contractReviewClaims, which skips unreadable rows. Pins (record-recognisers): (i) 「⭐ an unreadable hold line NEWER than a readable hold on this head voids nothing」: thread = readable hold (id 1) + a later comment quoting the template at a line start (id 7701) reads live off id 1 with unreadable [7701]; the in-case control leg, the same thread without 7701, reads the same id, session, condition and age with unreadable []. (ii) 「⭐ ONLY unreadable hold lines ⇒ unreadable, holding nothing」: two unreadable lines (7700, 7701) read unreadable, both named in gaps, the reading naming the newest (7701). (iii) 「⭐ PER HEAD — a newer readable hold naming an EARLIER head does not clear the older hold on this head」: reads live off the hold that names the current head, in either order (id 1 age 240 min; id 2 age 180 min). Rule implemented: per head, only a new head clears; among readable holds naming the current head the newest is the reading; if none names it, the newest readable hold reads stale. Plus: newest governs among holds on this head; every readable hold on earlier heads reads stale off the newest; every reading carries `unreadable` (empty when absent, listed beside stale). The old 'NEWEST governs … reads stale' case and the old 'unreadable NEWEST line makes the reading unreadable' case were replaced by (iii) and (i)+(ii). Pin (check-owed-reviews, row level): 「⭐ an unreadable hold line NEWER than a live hold on this head voids nothing」: the row stays WAIT, trigger 'none yet', hold live id 6472451890 holding '(b) waited 61 min', hold.unreadable [7701], renderRow prints 'until the last sibling PR merges ⚠️ 1 unreadable hold line(s) (7701)'; control = the battery's existing `held` row (no such line) is WAIT with no 'unreadable' text. No ablation (os-dev.md: scripts are not ablated; the order says the same). Old-path evidence by reasoning plus a read-only probe that mutates no file: base 167b0027 resolved by latestMarkedComment over all rows, so the newest marked comment (7701, unreadable) made the whole reading 'unreadable' and owedReviewRow let (b) fire (hold.state !== 'live'). Probe: `git archive 167b0027 scripts` extracted into the scratchpad, both versions imported side by side on fixture (i): control (readable hold only): base live id=1, branch live id=1 unreadable=[]; (i): base state=unreadable id=7701, branch state=live id=1 unreadable=[7701]. "Should-fix 2 premise measured false in its enumerated form: 'this file + contract-review.md + state-machine.md L40 + landing-operations.md 约定③' is NOT the whole remit. Other clauses also assign triage duties: dispatch-runbook.md L8 (pm:blocking in the triage sweep) and its sections 〈紧急卡直接分诊〉 〈分诊席读数成本三则〉 〈分诊两级盘点细则〉; cross-repo-coordination.md rules 1–2, L31, L36–37, L47; external-contributions.md L6; filing-gate.md L29, L34; landing-operations.md L83; seat-post-protocol.md L73; lanes/triage.md (the seat charter). So the restored bound is closed-world: this file plus the clauses elsewhere in the skill that name triage, with the three accepted homes named as examples. It is true by construction and still bounds the seat to written duties.",

State: card closed by the merge (Fixes); pm:dispatched stripped 2026-10-29T10:08Z; assignee kept as the record of who carried it. Open for the maintainer (not built here): should-fix 4 — whether trigger (c) also yields to a live hold (A: L30 states (a)(c) still fire; B: a live hold under 24 h suppresses (c) too; the seat recommends A). Noted, not filed: lanes/triage.md L7 vs triage-duties.md L24 ordering drift.
