feat: box.agent beats every 5 s, floor holds in every tenant (v0.13.1) - #30

box.agent's DefaultInterval and MinInterval are now 5 s, and the new Manifest.TenantFloorExempt makes that floor apply in every tenant, so a tenant's 30 s or 60 s min_check_interval_seconds no longer blocks box provisioning (the customer API cannot change it). missed_count defaults to 3 (silence after 15 s). The ingest rate limit goes back to 1/s. scripts/box-monitor-token.sh creates, rotates and deletes a box's monitoring token through the PlusClouds API.
