From #4520 (comment), it was actually not clear how the use of AI should be disclosed.
In Numpy's own issue template, they opted to add a section with a comment explaining how the disclosure should be done.
In this PR, I updated our issue template along the same pattern but thinned down the comment since I found it a little too verbose.
